there are surgical approaches that are sometimes helpful but these are only appropriate for a very few patients as they are invasive and involve damaging parts of the brain. i don't know of any dramatically new drugs not he horizon but glutaminergic agents like Namenda, Riluzole, and NAC are added to SSRIs with good effect in some patients. for mental rituals and obsessions, there is a good book called “The Imp of the Mind” by Lee Baer which may be very helpful.
